What’s the difference between detox, rehab, and therapy?
Detox, rehab, and therapy all play different roles in treating substance use problems. A person may need one, two, or all three, depending on their situation.
Detox
Purpose: Manage withdrawal safely when stopping a substance.
- Focuses on the physical effects of withdrawal.
- May involve medical monitoring, medications, and supportive care.
- Usually lasts days to a couple of weeks, depending on the substance.
- Does not by itself address the underlying reasons for substance use or teach long-term recovery skills.
Detox is often the first step, not the complete treatment.
Rehab (Rehabilitation)
Purpose: Help people stop using substances and build a sustainable recovery plan.
- Can be inpatient/residential (living at a treatment facility) or outpatient (attending treatment while living at home).
- Typically includes counselling, education, relapse-prevention planning, support groups, and skill-building.
- Focuses on changing behaviours and routines that contribute to substance use.
Rehab is generally more intensive and structured than standard therapy.
Therapy (Counselling/Psychotherapy)
Purpose: Address thoughts, emotions, behaviours, and life circumstances related to substance use.
- Can be individual, group, family, or couples therapy.
- Helps with cravings, coping skills, stress management, motivation, trauma, anxiety, depression, and relationship issues.
- May be used on its own for milder problems or as part of a broader treatment plan.
Common approaches include cognitive behavioural therapy (CBT), motivational interviewing, and other evidence-based treatments.
A simple way to think about it
- Detox: Helps your body safely stop using.
- Rehab: Provides a structured recovery program.
- Therapy: Helps you understand and change the patterns that contribute to substance use.
Example pathways
- Someone with mild alcohol misuse might benefit from therapy and support groups without rehab.
- Someone with alcohol dependence and withdrawal symptoms may need medical detox followed by therapy or rehab.
- Someone with repeated relapses and major life disruption may benefit from detox, rehab, and ongoing therapy.
